29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached: Jobsite - On or about April 2, 2021, an employee installing shingles on the roof did not have a lifeline connected to an anchorage capable of supporting 5,000 pounds where the lifeline was draped over the peak of the roof and not connected to anything.
