29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: At locations including but not limited to: a. On or about 9/11/2019, in the parts department, workers at the workstation were exposed to electrical shock and burn hazards where the employer used a metallic quad outlet designed for mounting as a pendant. b. On or about 9/11/2019, in the parts department, workers at the workstation were exposed to electrical shock and burn hazards where the employer "daisy chained" a relocatable 15A power strip from the improper pendant. The strip was powering two monitors, a copier, and two printers. There was also the availability for the use of personal fans, space heaters, and a vacuum cleaner which would be powered by the same strip. c. On or about 9/11/2019, in the receiving manager's office area, workers at the workstation were exposed to electrical shock and burn hazards where the employer used a metallic quad outlet to power the monitors, printers and electrical devices. d. On or about 9/11/2019, in the receiving manager's office area, workers at the workstation were exposed to electrical shock and burn hazards where the employer used a metallic quad outlet to power the monitors, printers and electrical devices.
