29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed: (a-c) Warehouse, southeast wall: On or about February 17, 2016, employees were exposed to electrical hazards, from a circuit breaker panel labeled 220 volts that had 3 open knockouts missing on the right side of the panel, exposing live electrical parts. (d) Warehouse, southeast wall: On or about February 17, 2016, employees were exposed to electrical hazards, from a breaker panel that had 1 open knockout missing on the right side of the panel, exposing live electrical parts. (e) Warehouse, southeast wall: On or about February 17, 2016, employees were exposed to electrical hazards, from an electrical control switch on the far left side of the electrical wall that had 1 open knockout exposing, exposing electrical parts. (f, g) Maintenance parts room, southeast wall: On or about February 17, 2016, employees were exposed to electrical hazards, from a 4 gang electrical outlet that had 2 open knockouts missing, exposing live electrical parts.
