29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): On or about 4/20/2015 at the Mattison grinder; employees performing grinding operations and cleaning operations were exposed to amputation hazards where: a. the four belt, 6 inch pulley that was 19 inches above the floor was not guarded, b. the one belt, 9 inch pulley that was 3 inches above the floor was not guarded.
