29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions: On about February 9, 2015 and in times prior thereto, a 460 VAC conductor cable entering a hole in the cabinet of a Kompressor, DAL 1310150845-001 did not have a bushing guard. This electrical hazard was located in the outside utility area.
