29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: On or about October 24, 2014 and prior to that time located at various points on the water tower powered single-point and two-point adjustable suspension scaffolds were utilized by employees who were not trained by a qualified person on the applicable standards or procedures to be followed to minimize scaffolding hazards.
