29 CFR 1910.266(i)(1): The employer did not provide training for each employee, including supervisors, at no cost to the employee: a: Rock Castle logging site. The employer did not provide training to employees involved in logging operations to ensure employees were capable of recognizing, preventing, and controlling safety hazards in the workplace. On February 21, 2013, a lodged tree was not felled or removed safely before continuing felling operations in the area which resulted in an employee being struck and killed by a 70' tall, 22 inch diameter white oak tree, as determined on March 13, 2013.
