29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: At the establishment, the employer had not conducted a worksite hazard assessment for the Maintenance and Refrigeration Department to determine if there were hazards present that would necessitate the use of Personal Protective Equipment (PPE) such as but not limited to footwear, SCBAs, protective level-A suits, air purifying respirators, etc. Employees were exposed to catastrophic release of ammonia which result in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
