29 CFR 1910.119(f)(3): The operating procedures were not reviewed as often as ne cessary to assure that they reflect current operating practice, including change s that result from changes in process chemicals, technology, and equipment, or changes to facilities: The employer does not ensure that operating procedures are review ed as often as necessary to reflect the current operating practice. a) This viol ation was observed on or about November 15, 2010, when it was determined that th e employer had last certified the Utility Pump House Operating procedures on Jan uary 28, 2003. This was over 6 years later than the due date. This exposed emplo yees to fire and explosion hazards. Pursuant to 29 CFR 1903.19(d), within 10 cal endar days of the date of the citation, the employer must submit documentation s howing that it is in compliance with the standard including describing the steps it is taking to ensure that operating procedures are reviewed and certified ann ually.
