29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information in paragraph (d) of this section) before further use or in a safe and timely manner when necessary means are taken to assure safe operation. a)On or about October 4, 2010 and at times prior thereto, failed ammonia detectors were not replaced in a timely manner. This deficiency was documented on February 17, 2010. b)On or about October 4, 2010 and at times prior thereto, inadequate ammonia pipe labeling was not corrected. This deficiency was identified on February 17, 2010. Note: Abatement certification and supporting documentation are required for this item.
