29 CFR 1910.119(l)(1) The employer did not establish and implement written proce dures to manage changes (except for "replacements in kind") to process chemicals , technology, equipment, and procedures; and, changes to facilities that affect a covered process. This violation was observed on or about May 7, 2010 when it was noted that th ere was no management of change (MOC) for the workaround that tied the SRU tail gas line into the flare line. This allowed flare gas containing light hydrocarbo ns to enter the thermal oxidizer and it led to the explosion in the Belco Unit o n February 5, 2010. This condition exposes employees to fire and explosion hazards. Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation that it is in compliance with the standard, including describing the steps it is taking to ens ure MOCs are conducted for all changes (except for "replacements in kind") to pr ocess chemicals, technology, equipment, and procedures; and, changes to faciliti es that affect a covered process.
