(b)The deadlegs/inspection ports on line 6"-PFF-401 were identified with thickness measurement locations but were not inspected on piping inspection reports TKF-RPT- PP180-014, 6/4/08 and MTW-A2350-14, 9/28/09. (c)The HF Hose Adapter Unloading Assembly connection comprised of 1" schedule 80 pipe threads, union, and bushing leaked on 3/10/11. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
