This violation most recently occurred at the Pasadena Refining Systems Inc. located at 111 Red Bluff Road, Pasadena, Texas on or about June 30, 2010, and at times thereafter, on the Alky 1, Alky 2, and FCC Units, exposing employee to potential fire and explosion hazards.
