29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about March 18, 2025, the employer did not ensure that the point of operation on the Wolf Stone Equipment Bridge Saw (Model No. ROTO545, S/N: XAH20210328) was effectively guarded. Employees enter into the frame of the bridge saw while it was in operation to manipulate slabs. This condition exposed employees to amputation and laceration hazards.
