29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: On or about November 4, 2024, and at times prior thereto and thereafter, ladders used by employees to perform utility work inside of trenches were not utilized how they were designed in the following instances a - c. a) On or about October 24, 2024, two (2) extension ladders were lashed together and used to access a trench by employees performing boring work; b) On or about November 1, 2024, an A-frame ladder was leaned against the trench face and used to access a trench by employees performing utility work; and c) On or about March 5, 2025, a two (2) piece extension ladder was separated, and the top extension section was used to access a trench by employees performing utility work.
