29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) At 20510 East Levy Street Williston, FL 32696, Log Shaver area; On or about 09/27/2024, the employer exposed employees to struck-by and caught-in hazards, in that the employer did not ensure the employees had knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the pneumatic energy before employees conducted servicing and maintenance activities on the log deck of log shaver machine 3. As a result, the remaining applicable energy control elements, involving machine shutdown [(d)(2)], machine isolation [(d)(3)], LOTO device application [(d)(4)], stored energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to safeguard employees from the machine servicing hazards. b) At 20510 East Levy Street Williston, FL 32696, Log Shaver area; On or about 09/27/2024, the employer exposed employees to struck-by and caught-in hazards, in that the employer did not ensure lockout devices were affixed to the motor disconnects by the employees conducting the servicing and maintenance activities at log shaver machines 1, 2, and 3. As a result, the remaining applicable energy control elements, stored energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to safeguard employees from the machine servicing hazards.
