29 CFR 1915.158(b)(5): There was not at least one portable or permanent ladder in the vicinity of each floating vessel on which work was being performed. On or about March 15, 2024, onboard the Cuyahoga vessel moored at the Port of Ashtabula, Kinder Morgan Pinney Dock; Minnesota slip, A/B dock side, the employer failed to provide employees with a ladder to use to exit the water if an employee fell into the water, exposing employees to hypothermia.
