29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. On or about March 28th 2024, an employee engaged in grain and material handling operations located at 1012 Laramie Ave, Hemingford, Nebraska 69348 were exposed to struck-by hazards in that the employer did not provide required forklift training that included formal instruction, practical training and performance evaluation prior to allowing employees to operate a forklift. Instances of include but are not limited to: i. Employee did not receive formal instruction. ii. Employee did not receive practical training. iii. Employee did not adequate a hands on performance evaluation.
