29 CFR 1910.146(f)(8): The entry permit that documented compliance and authorized entry to a permit space did not identify the measures used to isolate the permit space and to eliminate or control permit space hazards before entry: (a) On or about August 23, 2023, employees working in dryer C to retrieve derailed carts were exposed to a burn hazard in that the metal rails and carts in the dryer are exposed to heat up to approximately 275 degrees Fahrenheit and the measures used to isolate the permit space and to eliminate or control the burn hazard was not identified on the entry permit; (b) on or about August 23, 2023, employees working in dryer C to retrieve derailed carts were exposed to a struck-by hazard from carts being unexpectedly pushed into the dryer and the measures used to isolate the permit space and to eliminate or control the struck-by hazard was not identified on the entry permit; (c) On or about August 23, 2023, employees working in dryer C were exposed to the hazard of being struck-by and/or entrapped by the exit door which is raised to allow entry into the dryer and the measures used to isolate the permit space and to eliminate or control the struck-by and/or entrapped-by hazard was not identified on the entry permit.
