29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) in a safe and timely manner when necessary means were not taken to assure safe operation: a) Polyphase Part II & methanol recovery process Several valves upstream to the mother liquor tank T-7119 were severely corroded, including with missing valve handles, but in methanol service. Valves included VP-7370, VP-7369, VP-7367, and an unidentified 1-inch ball valve between VP-7101 and VP-7367. Violation occurred on or about 7/12/23.
