29 CFR 1910.305(b)(2)(i):Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: a) Center Manufacturing Company, on or about December 14, 2022, at the address of 540 Goodrich Rd. Bellevue, OH; Employees working in the pallet notching department were exposed to electrical shock when accessing the 240v receptacle to power the auger. The employer failed to ensure 240v receptacles face plate was properly installed.
