29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: Central Paper Stock, Inc. located at 6665 Jonas Place in Berkeley, MO 63134: The employer is failing to protect employees from caught-in and crushing hazards while performing maintenance, clearing a jam or cleaning machines. This condition was most recently observed on or about 12/9/2021, when the employer failed to develop equipment specific energy control procedures while clearing jams, cleaning or performing maintenance on machines such as, but not limited to, the following: a) The large "Harris Selco" baler (Model: HLO-8221AR-150; Serial No: 39770079) on the paper side of the plant; b) The small "Harris Selco" baler (Model:L125-2-10/8 ; Serial No:08138825 ) on the paper side of the plant; c) The "Retech" book grinder (Model: RG 52; Serial No: 7739101) on the paper side of the plant; and, d) The plastic "Badger" baler (Model: BADGER L-100S-2-10/7; Serial No: 8658) on the plastic side of the plant. Amended to: The classification of Citation 1, Item 1O as herein amended shall remain serious as issued. The AVD is amended to include as follows: Central Paper Stock, lnc. located at 6665 Jonas Place in Berkeley, MO 63134: 1. The employer is failing to protect employees from caught-in and crushing hazards while performing maintenance, clearing a jam or cleaning machines. This condition was most recently observed on or about 12/9/2021, when the employer failed to develop equipment specific energy control procedures while clearing jams, cleaning or performing maintenance on machines such as, but not limited to, the following: a) The large "Harris Selca" baler (Model: HLO-8221 AR-150; Serial No: 39770079) on the paper side of the plant; b) The small "Harris Selca" baler (Model:LI 25-2-10/8 ; Serial No:08138825) on the paper side of the plant; c) The "Retech" book grinder (Model: RG 52; Serial No: 7739101) on the paper side of the plant; and, d) The plastic "Badger" baler (Model: BADGER L-I00S-2-10/7; Serial No: 8658) on the plastic side of the plant. 2. The employer is failing to protect employees from caught-in and crushing hazards while performing maintenance, clearing jams or cleaning machines. This condition was most recently observed on or about 10/26/2021. The employer provided three locks to lockout equipment in the facility. The locks shared a common key and were not assigned any specific employee. Employees routinely used these locks to lock out machines such as, but not limited to, the following: a) The large "Harris Selco" baler (Model: HLO-8221 AR-150; Serial No: 39770079) on the paper side of the plant; b) The small "Harris Selco" baler (Model:Ll25-2-I0/8 ; Serial No:08138825) on the paper side of the plant; c) The "Retech" book grinder (Model: RG 52; Serial No: 7739101) on the paper side of the plant; and, d) The plastic "Badger" baler (Model: BADGER L-100S-2-10/7; Serial No: 8658) on the plastic side of the plant
