29 CFR 119.(j)(4)(iv) The employer did not document each inspection and test that had been performed on process equipment with documentation that identified the date of the inspection or test, the name of the person who performed the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test performed, and the results of the inspection or test. a) Chlorine process: The employer did not document annual inspections of the H and M evaporators that included a description of the inspection or test, the results and the person who performed the test. This included testing for equipment, such as but not limited to, electrical system checks of relief system pressure switches, automatic water level control and low level switch and operational testing of pressure relief systems and in place cleaning of the evaporator cylinder the system according to the manufacture, on or about 8/10/21. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
