29 CFR 1915.89(b): The employer did not establish and implement a written program and procedures for lock out and tag out systems to control hazardous energy during the servicing of any machinery, equipment, or system in shipyard employment to include requirements of items 1 through 6 of this paragraph: A) 7 Mosquito Lane, Tuckahoe, NJ: The employer failed to ensure that a Lock Out Tag Out program was established, and all employee were trained to the program exposing them to electrical shock, burns and electrocution. Condition noted on or about 23JAN20 NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
