29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Sawmill - On or about October 16, 2019 lockout devices were not applied to all energy sources while repairs were made to the head rig exposing employees to struck-by and amputation hazards. (b) Sawmill - On or about October 16, 2019 employees change the blades on chipper without locking energy sources exposing employee to struck-by and amputation hazards.
