29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) On September 7, 2018 employees were exposed to crush-by hazards from operating a Toyota forklift model 8FGU25, serial number 68430 with a broken seat belt. The forklift had a seat belt tongue but was missing the seat belt buckle.
