29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures, or by any of the following means identified in sub-paragraphs (A)through (D) of this paragraph: On or about August 09, 2017, the employer failed to ensure that a 240 volts, 3phase service panel located on the north wall, was effectively closed and/or free from damage. The service panel had several unused circuit breaker opening, exposing the energized bus bar.
