29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained. a. On or about August 17, 2017, located at Table 4, Roll 4-21, the employer did not ensure that all employees involved in the removal of the 4-pack in order to remove the bearing housing to test motor were protected by their own locks affixed to a group lockout device, thereby exposing the employees to an unexpected startup of the equipment.
