29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by 29 CFR 1910.178(l): (a) On or about April 14, 2017, and times prior thereto throughout Plant #3, employees operated Hyster 30 Fortis forklift trucks without having first completed training consisting of formal instruction, practical training, and an evaluation of the operator's performance in the workplace. Employees were exposed to struck-by and/or crushed by hazards.
