29 CFR 1910.303(f)(4): Disconnecting means were not capable of accepting a lock in the open position. On or about April 20th, maintenance employees were exposed to electrical shock hazards due to the following equipment being damaged: Instance A: In the Iron Press Area, the 460 volt panel for Iron 3 had a broken electrical disconnect and was not capable of accepting a lock. Instance B: In the Mat Cell Area, the main electrical panel for Washer 1 had a broken disconnect and was not capable of accepting a lock.
