29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) On or about July 7, 2016, the employer failed to ensure that established procedure for the application of energy control covered the actions listed in and was done in sequence as required by 29 CFR 1910.147(d)(1)-(6). Employees were exposed to caught in and electric shock hazards in that employees performing operations in which elbow machine sizing components were changed and lubricant reservoirs were refilled did not follow an established procedure for energy control. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
