Facility - On or about 5/14/16, employees removed an ~ 15 x 7 floor section of flooring that contained a layer of ACM surfacing materials (25% chrysotile) between the two layers of wood. The materials from the removal were not collected and disposed of in sealed, label impermeable bags or other closed, labeled impermeable containers. NO ABATEMENT CERTIFICATION REQUIRED
