29 CFR 1910.305(d)(2): Panel boards were not mounted in cabinets, cutout boxes, or enclosures approved for the purpose and were not dead front: a) Grinder Electrical Room; On or about March 15, 2016 and at times prior thereto the employer exposed employees to electrical hazards in that employees were allowed to operate breakers mounted in a cabinet with the front cover removed and was not dead front exposing live unguarded 480 volts 3 phase AC electrical parts.
