29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): Shop: On, or about, August 18, 2015, the company had not ensured that all forktruck operators were trained and competent to operate forktrucks. An operator was observed operating a forktruck without wearing the seat belt provided and the electrical system provided for the shredder had suffered major damage on several incidents involving forktrucks.
