29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. At the workplace, the Accurshear, model 625010, serial # 5376, had a barrier guard with a gap beneath the guard allowing access to the point of operation, thereby exposing employees to an amputation hazard. b. At the workplace, the Accurpress #1, model 7608, serial # 5238, did not have the point of operation adequately guarded, thereby exposing employees to an amputation hazard. c. At the workplace, the Accurpress #2, model 7608, serial # 4639 , did not have the point of operation adequately guarded, thereby exposing employees to an amputation hazard. d. At the workplace, the Accurpress, model 7606, serial # 5737, did not have the point of operation adequately guarded, thereby exposing employees to an amputation hazard. e. At the workplace, the Accurpress, model 76010, serial # 7385, did not have the point of operation adequately guarded, thereby exposing employees to an amputation hazard. f. At the workplace, on the ingoing side of roll form #1, the flattening rollers did not have the point of operation guarded, thereby exposing employees to a caught-in hazard. g. At the workplace, on roll form #1, employees were not protected from the point of operation while making machine adjustments, thereby exposing employees to a caught-in hazard. h. At the workplace, in the maintenance shop, a Bridgeport vertical mill, did not have the point of operation adequately guarded, thereby exposing employees to an amputation hazard.
