29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about March 17, 2015, in Tool & Die Shop of Building #1, employees were exposed to struck-by and caught-between hazards such as those created by point of operation, ingoing nip points, rotating parts, flying abrasive chips and sparks in the following instances: (a) The Chevalier Surface Grinder (Model FSG-618M) was missing its protective abrasive wheel guard. (b) The Okamoto Surface Grinder (Model Linear 612) was missing its protective abrasive wheel guard.
