29 CFR 1910.252(a)(3)(i): Used containers. No welding, cutting, or other hot work shall be performed on used drums, barrels, tanks or other containers until they have been cleaned so thoroughly as to make absolutely certain that there are no flammable materials present or any substances such as greases, tars, acids, or other materials which when subjected to heat, might produce flammable or toxic vapors. Any pipe lines or connections to the drum or vessel shall be disconnected or blanked. a. On or about November 6, 2014, located in the tank yard at Rhodes Manufacturing, employees performed welding and cutting on used crude oil storage tanks that had not been so thoroughly cleaned as to make absolutely certain that there were no flammable materials present or any substances such as greases, tars, acids, or other materials which when subjected to heat, which would produce flammable vapors. The employer failed to utilize a system, i.e. flammable gas detector with an oxygen analyzer that would ensure that the oxygen and flammable levels were displaced and essentially zero. The employees were exposed to an explosion hazard by not ensuring that the atmosphere in the tanks was safe for hot work to commence.
