29 CFR 1926.1206(i) The entry permit that documented compliance and authorized entry to a permit space did not identify the measures used to isolate the permit space and to eliminate or control permit space hazards before entry: a) 9000 River Rd, Delair, NJ, 08110- Anodizing: A permit completed for entry into an anodizing tank for the installation of a stainless steel trough did not identify the measures used to eliminate and control the hazards such as, but not limited to, proper ladder selection and use for entry, ventilation for atmospheric hazards and blanking/blinding of pipes containing chemicals and water, on or about 2/24/2017. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
