29 CFR 1926.1206(h) The entry permit that documented compliance and authorized entry to a permit space did not identify the hazards of the permit space to be entered: a) 9000 River Rd, Delair, NJ, 08110- Anodizing: A permit completed for entry into an anodizing tank for the installation of a stainless steel trough did not list the hazards of the space including, but not limited to engulfment from water and chemicals, burns from chemicals, falls during entry and oxygen deficiency, on or about 2/24/2017. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
