ORS 654.010: The employer did not furnish a place of employment which was safe and healthful for employees, did not furnish and use such devices and safeguards, and did not adopt and use such practices, means, methods, operations and processes as was reasonably necessary to render such employment and places of employment safe and healthful, and did not do every other thing reasonably necessary to protect the life, safety, and health of the employees: a) As of the date of the inspection, the employer failed to furnish reasonably necessary devices and safeguards or did not adopt and use practices, means, methods, operations, and processes as stipulated in industry standards such as, but not limited to, the American National Standards Institute/American Society Heating, Refrigerating and Air Conditioning Engineers (ANSI/ASHRAE) and the International Institute of Ammonia Refrigeration (IIAR). As shown by one or more of the following: i) The employer failed to maintain the ammonia detection system, installed in the 2008 ammonia engine room, according to the manufacturer's recommendations and the industry standards. (See ANSI/ASHRAE Standard 15 - 2001 Safety Systems for Refrigeration Systems, Section 11.6.3 - Periodic Tests) ii) The employer failed to maintain isolation valves installed in the 2008 ammonia system as stated by the industry standards of ANSI/ASHRAE and IIAR. (See ANSI/ASHRAE Standard 15 - 2001 Safety Systems for Refrigeration System, Section 11.6; See International Institute of Ammonia Refrigeration (IIAR) Bulletin 110 Revised 03/02: Guidelines for Start-up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems, Section 6.6.1 Shut off valves) iii) The employer failed to maintain the condensers servicing the 2008 refrigeration system according to the manufacturer's recommendations. (See Evapco Manufacturer's Manual and Bulletin 112AC: Maintenance Instructions Closed Circuit Coolers and Evaporative Condensers) iv) The employer failed to maintain safeguards such as equipment manuals, current piping and instrument drawings, preventative maintenance schedules, and energy isolation procedures for the 2008 ammonia system.
