29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: On or about August 14th, 2024, employees engaged in production activities in the commercial baking facility located at 4657 G Street, Omaha, Nebraska 68117 were exposed to caught-in and nip point hazards in that the employer failed to ensure funning chains and sprockets are guarded against accidental and/or incidental contact. Instances of failure, include but are not limited to: a) Unguarded Red Freezer Chain and Sprocket. b) Unguarded Blue Freezer Chain and Sprocket. c) Unguarded chains and Sprockets on the Blue Oven. d) Unguarded chain and sprocket next to the icing tank on panning line below the conveyor.
