29 CFR 1910.335(a)(2)(i)(A): Fuse handling equipment, insulated for the circuit voltage was not used to remove or install fuses when fuse terminals were energized. a. PM #9, Drive Room 2- On or about November 27, 2019, an employee opening the 2400 volt fused disconnect for the #2 Jones Refiner was not protected from an electric shock hazard where a wooden stick, that was not rated for 2400 volts, was used to open the fused disconnect. b. PM #9, Drive Room 1- An employee opening the 2400 volt fused disconnect for the Spare Press Vacuum Pump was not protected from an electric shock hazard where a wooden stick, that was not rated for 2400 volts, was used to open the fused disconnect.
