29 CFR 1910.178(l)(6):The employer did not maintain the certification records for employees operating a powered industrial truck. The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. : UTCRAS Inc., located at 501 Highland Ave. Morton, PA: (a) UTCRAS, Machine shop, Throughout the facility: On or about January 21, 2021, the employer did not maintain certification records for employees using propane powered trucks such as , but not limited to, 80 Nissan (Model: MCUG1F2F361 V) and Hyundai (Model: 25L-7A) and Mitsubishi Caterpillar (Model: GC30K1). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.
