29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves. a). On or about April 12, 2019, the employer did not ensure a projecting shaft edge on the rear of the Extruder Line #1 extruder machine had a smooth edge or was guarded by a nonrotating cap or safety sleeve. b). On or about April 12, 2019, the employer did not ensure a projecting shaft end on the rear of the Extruder Line #2 extruder machine had a smooth end or was guarded by a nonrotating cap or safety sleeve. c). On or about April 12, 2019, the employer did not ensure a projecting shaft edge on the rear of the Extruder Line #3 extruder machine had a smooth edge or was guarded by a nonrotating cap or safety sleeve. d). On or about April 12, 2019, the employer did not ensure two projecting shaft edges and ends of the lacquer take up machine were guarded by a nonrotating cap or safety sleeve.
