29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service: 11 Cody Street: On, or about February 8, 2017, in the following locations, flexible cords were damaged: A. Booth 7: An orange and black extension cord used to energize the Bugo traverse unit was not grounded. B. Booth 3: The power cord provided on the Plasmatron powder feed unit had two damaged areas on the outer insulation and the power cord provided on the heavy duty turn table had a damaged area on the outer insulation. C. Booth 1: The power cord provided on the turntable had damage on the outer insulation and was wrapped with masking tape.
