29 CFR 1910.272(k): Receiving pit feed openings in the grain handling facility, such as truck or railcar receiving pits, were not covered by grates which had a maximum opening width of 2 2-Jan inches: (a) Middle Pit Area, on or about and at times preceding to September 9, 2015, when unloading cottonseed, the pit grate was removed, exposing employees to amputation hazards from the operating auger during cleanup operations around the pit.
