29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs seven feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt. On 5/27/15, at the Washington facility, employees were exposed to amputation hazards as belts - when running with pulleys - were not guarded. Instances included: 1 Outfeed belt conveyor motor 2 Planer Mill conveyor motor high speed 3 Planer Mill Infeed - high speed 17 ¼ inch opening 4 Planer Mill Infeed - where lumber enters 1 log/time 5 Planer Mill West side of building - 2 motors with exposed belt and pulley 6 Stacker Pit - On underside at pit area - where load exits In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
