29 CFR 1910.147(d)(5): Following the application of lockout devices to energy isolating devices, all potentially hazardous stored or residual energy was not relieved, disconnected, restrained, or otherwise rendered safe. And where there was a possibility of reaccumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation no longer existed: On or about August 28, 2014, All energy sources were not relieved and/or disconnected when employees were required to service, setup or perform maintenance on machinery and/or equipment. Employees were exposed to electrocution and amputation hazards.
