29 CFR 1910.119(f)(1): The employer did not develop and implement written operat ing procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and which addressed the elements listed in 29 CFR 1910.119(f)(1) (i) through (f )(1)(iv): The employer does not ensure that operating procedures are written to provide clear instructions for safely conducting activities involved in each covered process. a) This viola tion was documented on or about November 18, 2010 in the BD Recovery Area of Lio n Copolymer - Baton Rouge, when the employer allowed and/or required that critic al valves in the Butadiene Recovery process be operated using come-alongs, witho ut ensuring that the procedures that are in place reflect the current operating practice. b) This violation was documented on or about November 18, 2010 in the BD Recovery Area of Lion Copolymer - Baton Rouge when the employer did not modif y procedure LC.12.204 section 5.5.3 (Bates # LION0008288) of the Recovery Area O perating Procedures Manual to remove reference to the steam driven Butadiene com pressor, which has been retired from the process. c) This violation was document ed on or about November 18, 2010 in the BD Recovery Area of Lion Copolymer - Bat on Rouge when the employer did not modify procedure LC.12.204 section 6.4 (Bates # LION0008297) of the Recovery Area Operating Procedures Manual to in clude current operator position of Flex Operator. d) This violation was document ed on or about November 18, 2010 in the BD Recovery Area of Lion Copolymer - Bat on Rouge when the employer did not modify procedure LC.12.204 sections 8.3.1.10 and 8.3.1.25 (Bates # LION0008355) of the Recovery Area Operating Procedures Man ual to include current operator position of Flex Operator. e) This violation was documented on or about November 18, 2010 in the BD Recovery Area of Lion Copoly mer - Baton Rouge when the employer did not modify procedure LC.12.204 section 8 .3.2 (Bates # LION0008356) of the Recovery Area Operating Procedures Manual to i nclude current operator position of Flex Operator. f) This violation was documen ted on or about November 18, 2010 in the BD Recovery Area of Lion Copolymer - Ba ton Rouge when the employer did not modify the emergency procedure LC.12.204 section 10.4.5 (Bates # LION0008390) of the Recovery Area Op erating Procedures Manual to account for the steam butadiene compressor (Compres sor #1) being retired. g) This violation was documented on or about November 18, 2010 in the BD Recovery Area of Lion Copolymer - Baton Rouge when the employer failed to provide clear instruction for sampling Acrylonitrile in LC.12.204 sect ion 6.8.2 of the Recovery Area Operating Procedures Manual. This exposed employe es to fire and explosion hazards.
