29 CFR 1910.119(e)(6): The employer did not ensure that the process hazard analysis was consistent with the current process: ALON USA, LP, 200 Refinery Road, Big Spring, TX. The employer does not ensure the process hazard analysis is updated to accurately reflect existing technology being used to provide early warning of releases of toxic and flammable materials. This violation was observed on or about April 15, 2009 in the following instance: The most recent process hazard analysis (PHA) for the #1 Sulfur Recovery Unit (SRU) and Shell Claus Offgas Treating (SCOT) Unit, completed in May 2009, did not address detection technologies that would provide early warning of releases of a toxic and flammable material, such as hydrogen sulfide released from leakage of process streams. Thus employees are not aware of potentially hazardous concentrations of hydrogen sulfide which might cause a fire and / or explosion, or asphyxiation and death. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking in the workplace to ensure the process hazard analysis is updated to accurately reflect existing technology being used to provide early warning of releases of toxic and flammable materials.
